Instruction: 3 ways to send forms
Forms are available to the left of this text (under Forms On-line heading) for download and completion. You may choose from one of the following 3 methods to send the form.
1.E-mail it to the parish office at stbasilsparish@gmail.com.
Download the MS-Word version of the form and complete it in an MS-Word compatible word processor.
Download and print the MS-Word version or PDF version of the form. You may then choose to:
2.Fax it to the Parish office: 613-729-0498
3.Drop it off at the Parish office 940 Rex Ave, Ottawa, ON.
Please call us (613-729-5149) to confirm office hours if you wish someone to accept it in person.

3. Logo Download
Stephen Smith, a graphics designer based in Leicestershire England, has won the competition for the logo and has provided a few files for our use at St. Basil's below. The church he attends (since he was a child) is called All Saint's Church.
